Company identification cards issued by private businesses or institutions registered with, controlled, or regulated by the BSP, SEC, or IC can also be provided.

  • (Only for foreign nationals). Any one of these proofs of residency: alien certificate of registration, immigrant certification of registration, or passport copy of resident visa stamp

  • Proof of income, which includes all of the following:

    • For employed applicants:

      • Photocopy of the most recent ITR/BIR form 2316

      • Photocopy of the most recent three months' pay slips

      • The employment certificate states the salary, position, date employed, and ITR or pay slips

      • Valid credit card reference or the most recent credit card statement of account

      • Other documentation may be required for additional verification

    • For self-employed applicants:

      • Copy the most recent ITR/BIR form 1701 and audited financial statement

      • SEC registration or DTI registration certificate (for sole proprietorship)

      • A certificate with a page listing the applicant's shares (for partnerships or corporations)

      • Permit for business

      • Valid credit card information or the most recent credit card statement of account

      • Other documents, such as a Mayor's permit, may be required for further verification

      • Please keep in mind that all documents submitted/presented must be in the applicant's name (correct spelling required, include suffix if applicable)
